CaPE Name Change Policy

Changes to your first name or last name in University of Arizona Continuing & Professional Education (CaPE) records are only permitted while you are currently participating in a CaPE course or program.

If you are currently participating in a CaPE course or program, you can submit a written name change request via email, including the following information:

• Original first name
• Original last name
• New first name
• New last name
• Email address
• Name of your course/program

If you have already completed your course or program, we are unable to accept name change requests. We do not alter historical program participation records or re-issue certificates of completion to reflect name changes.
